Clinical team members Andrew Holt, PharmD, BCMAS, CPGx, and Savannah Roberts Clary, Pharm.D., BCPS, CPGx, were recently featured in Drug Topics magazine as contributors to a two-part series on tianeptine.If you are not familiar with tianeptine, you should know that it is easily accessible in most states under brand names like Tianaa, Neptune’s Fix, and more. Use of tianeptine alone or in combination with other substances or prescription drugs can complicate patient management and increase their risk of drug interactions and harm. Tianeptine’s activity at opioid receptors helped it gain its nickname of “gas station heroin.” Testing for tianeptine in the healthcare setting can help with educating patients, as they may not voluntarily report use, and it can also help inform treatment decisions. Last week, (CFSRE) published an alert regarding 2F-2oxo-PCE, a substance that has similar characteristics to ketamine. This is a drug that is not currently regulated in the United States, produces dissociative effects when used, and has been found in several states across the country.The drug was first identified in Australia, but has since been detected in both drug materials and toxicology samples. Use of this drug has been associated with both intoxication and fatalities, and it has been found alongside other potent, high-risk substances. The CSFRE report states that “…drug materials containing 2F-2oxo-PCE also contained opioids, primarily fentanyl. Xylazine, NPS benzodiazepines, and nitazene analogues were common co-occurrences alongside 2F-2oxo-PCE.”Aegis implemented testing for this drug in September 2023, and in cases where use is suspected, it can be tested in both urine and oral fluid specimens. Signs and symptoms of gabapentin misuse or abuse can include the development of tolerance, self-dose escalation, and drug-seeking behavior. Because trends in gabapentin misuse and diversion are especially seen in those with a history of opioid abuse, understanding risk factors of the patient before prescribing can reduce the rate of gabapentin abuse. Gabapentin taken at higher doses can produce varying desired effects, including improved sociability, increased energy, marijuana-like relaxation, or dissociation. Special attention must be given when prescribing gabapentin for patients who are also taking any drug that results in central nervous system depression. The most common classes include opiates, buprenorphine, benzodiazepines, and antihistamines. When these classes are used concomitantly with gabapentin, there is risk for respiratory depression, potentially resulting in death. In 2022, the FDA administered a warning that patients using gabapentin who have respiratory risk factors may have serious breathing difficulties when used with opioids or other CNS depressants. On May 20, 2024, the (CFSRE) distributed analertregarding proliferation of medetomidine in multiple parts of the country.In response to the alert, we wanted to share our findings from when Aegis began testing for medetomidine in Q2-2023 to help raise awareness of theof the potential consequences of this drug in communities and the potential additive risk of this substance when found with other drugs like illicitly-manufactured fentanyl, xylazine, or other central nervous system depressants.As substances within circulation continue to change rapidly, our goal is to remain diligent in updating testing methodologies to reflect drugs that are in use and provide clinicians an opportunity to identify at-risk individuals. Gabapentin can be efficacious when co-prescribed in the management of pain or pain-related conditions, AUD, and some forms of anxiety disorders. Still, gabapentin can be dangerous when used with CNS depressants or in high doses alone.
